Correction of name
If your name on the UTS student system:
- is incorrect, or
- has changed.
You should complete the form:
Correction of name application.
Correction of name applications will not be processed if submitted by fax.
Further instructions on completing and submitting this form are provided on the form.
Current students
After lodging your correction of name application, you should advise :
- all staff in classes that you attend, and
- subject co-ordinators of subjects you are currently taking or about to take
of your change of name
Refer to your new and former names in any correspondence and on exam scripts, for example:
- Jones (formerly Smith), or
- Jones (nee Smith).
